Free 5S / 6S Audit Template
Score Every Area. Close Every Finding. Sustain the Gains.
Sort, Set in Order, and Shine are the quick wins. Sustain is where most 5S programs fade, because findings get written down and never tracked.
What is a 5S audit?
A 5S audit is a regular, scored inspection of a work area against the five pillars of 5S: Sort, Set in Order, Shine, Standardize, and Sustain. A 6S audit adds Safety as a sixth pillar.
What's included
- 19 criteria, each with an evaluation guide, scored 0 to 4 or N/A
- An optional Safety (6S) section you can switch on or off
- Automatic section scores, an overall score, and a rating band
- An Action Plan tab with owners, due dates, and overdue flags
- A Score History tab to track trends by line and area
How to use it
Add your lines, areas, and shifts on the Lists tab. Walk the area, score each criterion, and log anything scored 0 to 2 as an action. Record each audit in Score History so you can see whether gains are holding.
Who it's for
Continuous improvement leads, area managers, and plant teams running 5S or 6S in manufacturing.

Build a Workplace That Stays Organized.
After a structured 5S rollout, one manufacturer's production-system efficiency rose from 67% to 88.8% (Ahire et al., 2021).
5S is usually a plant's first step into Lean. It creates a clean, organized, safe workplace where problems are visible and the right tool is always in the right place.
A steady audit cadence protects those gains and turns 5S from a one-time cleanup into a habit.
