Free BBS Observation Score Card
Observe It. Coach It. Fix It.
A good observation card takes minutes to fill out and leads to a real conversation, not a filed form.
What is a BBSO observation score card?
A BBSO (Behavior-Based Safety Observation) score card is the form an observer uses to record a short observation of a coworker's task. Each behavior is marked Safe or At-Risk, and the card captures the feedback conversation and any follow-up actions.
What's included
- 34 behaviors across six categories: Body Position, PPE, Tools & Equipment, Mobile Equipment & Traffic, Work Environment, and Procedures
- Safe and At-Risk checkboxes for every item, clickable in Word
- A feedback conversation checklist, including Stop Work
- A prompt to capture why an at-risk behavior happened
- Fields for kudos, countermeasures, an owner, and a due date
How to use it
Tell the worker you're observing and that no names are recorded. Mark only what you see, talk it through before you leave, and log any follow-up so it gets closed.

Find Risk at the Base of the Triangle.
Strong leading-indicator scores have been linked to reductions of up to 34% in recordable injury rates (American Journal of Industrial Medicine).
Behavior-based safety focuses on the actions that come before an injury. Regular, respectful observations show where shortcuts and workarounds are creeping in, and why.
With that insight, supervisors can fix the root cause, whether it's training, equipment, or a procedure that doesn't match the job.
