One Page. Six Categories. Every Shift.
Observers need a form they can complete on the floor in a few minutes, not a packet they'll avoid.
What is a BBS observation form?
A BBS (behavior-based safety) observation form is a checklist used to record safe and at-risk behaviors during a short, planned observation of work. It gives observers a consistent way to capture what they saw and what should happen next.
What should a BBS observation form include?
A good form covers the main behavior categories, gives a clear way to mark Safe or At-Risk, and prompts a conversation. This one includes:
- Body Position, PPE, Tools & Equipment, Mobile Equipment & Traffic, Work Environment, and Procedures
- Fields for observer, area, shift, task, and number of people observed
- Kudos, at-risk details, likely causes, and a proposed countermeasure
- A follow-up owner and due date
Paper or digital?
Paper forms are a good way to start. As observation volume grows, many teams move to digital forms, so submissions take seconds, supervisors get notified, and results update automatically.

Prevent Injuries Instead of Paying for Them.
U.S. employers pay more than $1 billion a week in direct workers' compensation costs for non-fatal injuries (Liberty Mutual).
A BBSO program shifts that effort toward prevention. Employees observe one another, reinforce what's working, and flag at-risk behavior before anyone gets hurt.
Because it's peer-to-peer and non-punitive, participation grows over time, and so does the data leaders need to act.
