See It. Evaluate It. Control It.
Conditions on the floor change by the hour: a new task, a contractor on site, a wet floor, a machine that needs to be locked out.
What is a Field Level Hazard Assessment?
A Field Level Hazard Assessment (FLHA) is a short written check completed at the work site before a task begins. Workers identify hazards, rate the risk, choose controls, and sign off that they understand the plan. It's also known as a field level risk assessment or pre-task hazard assessment.
What's included
- A hazard checklist across six categories: Physical, Energy & Equipment, Ergonomic, Chemical, Biological & Food Safety, and People & Psychosocial
- A color-coded risk matrix with clear stop-work thresholds
- A hazard and controls table with risk ratings before and after controls
- PPE, emergency information, and a working-alone check-in plan
- Pre-start checks for LOTO, permits, and nearby workers
- Worker sign-off and an end-of-task review
When to use it
Complete an FLHA before any non-routine task, maintenance job, or contractor work, and again whenever conditions change mid-task. It makes sure every hazard is named, controlled, and acknowledged by the people doing the work.

Assess Risk at the Point of Work.
Preventing a single major incident can save $100,000 to $250,000 or more (Institute for Work & Health).
Field level hazard assessments are one of the simplest ways to get there. A few minutes of structured thinking before a task catches the hazards a general procedure can't anticipate.
They also create a signed record that the risk was reviewed, which matters for contractors, maintenance work, and audits.
