Free Forklift Pre-Shift Inspection Checklist

Inspect Every Truck. Every Shift. Before It Moves.

A pre-shift inspection only works if it's done, done honestly, and acted on. Paper checklists that get signed without a real walkaround don't protect anyone.

What is a forklift pre-shift inspection?

A forklift pre-shift inspection is a check operators complete before using a powered industrial truck. OSHA requires trucks to be examined before being placed in service, and any truck with a defect that affects safe operation must be taken out of service until it's repaired (29 CFR 1910.178(q)(7)).

What's included

  • Visual checks with the key off: fluids, leaks, tires, forks, mast and chains, hoses, overhead guard, data plate, seat belt
  • Checks specific to electric and LPG or internal combustion trucks
  • Operational checks with the key on: horn, lights, backup alarm, steering, brakes, hydraulics, seat switch
  • OK and Defect checkboxes for every item, clickable in Word
  • A clear "safe to operate?" decision with tag-out instructions
  • Sign-off for the operator, supervisor, and repair

How to use it

Keep a stack with each truck or at the charging station. Operators complete one per truck, per shift, before the first lift. Any defect goes to the supervisor right away, and the truck stays parked until it's repaired and signed back into service.

Stop Unsafe Trucks Before They Move.

Powered industrial trucks (29 CFR 1910.178) regularly appear on OSHA's list of most frequently cited standards.

Most forklift problems show up before an incident: a soft brake, a leaking hose, a horn that doesn't work. A consistent pre-shift check catches them while the truck is still parked.

When defects are reported and fixed fast, operators trust the process and keep doing real inspections instead of signing a blank form.
